Quality Nonconformance and Corrective Action

Two thirds of corrective actions address a symptom and the failure returns within the quarter. Delegates practise structured investigation on customer complaints and production defects, using cause and effect analysis, the five whys done properly, and fault trees, then write actions that survive verification.

What you will be able to do

  • Separate the problem definition from the assumed cause before any investigation starts.
  • Gather production records, inspection data and operator accounts around a nonconformance.
  • Apply cause and effect diagrams, the five whys and fault tree analysis to the appropriate problem type.
  • Distinguish physical, human and system level causes so that actions reach the right layer.
  • Write corrective actions that are specific, owned, dated and testable.
  • Verify effectiveness after implementation and escalate when a fix has not worked.
