Patient Safety Incident Reporting and Quality of Care

Most adverse events are reported late, reported thinly, or not at all, and the ward learns nothing. Delegates build a working incident reporting and review system: classification and severity rating, root cause analysis, complaints handling, clinical audit cycles and quality indicators reported to management.

  • Classify patient safety incidents by type and severity using a consistent rating scale.
  • Build a reporting culture that separates organisational learning from disciplinary action.
  • Conduct a root cause analysis and produce corrective actions that can be tracked.
  • Handle patient complaints and compliments through a single case management process.
