Home Based and Palliative Care Services Coordination

Support managers and team leaders who coordinate home based and palliative care across South African communities where care extends well beyond the clinic. The course covers caseload allocation, home visit scheduling, family and caregiver support, medication and pain management coordination and referral pathways. Participants gain practical tools to organise dispersed care teams, manage limited transport and deliver dignified support to patients in their homes.
