Failure Mode and Effects Analysis Workshop

New products, modified processes, and imported equipment often reach South African production lines without a structured review of what could go wrong before customers or auditors find the problem first. This hands on workshop teaches design and process FMEA methodology, including severity, occurrence, and detection scoring, so engineering and quality teams can prioritise risks and build controls in before a failure ever reaches a customer.

What you will be able to do

  • Distinguish between design FMEA and process FMEA and when to apply each.
  • Identify potential failure modes for a product, process, or piece of equipment.
  • Score severity, occurrence, and detection to calculate a risk priority number.
  • Rank risks and agree which failure modes need controls before others.
  • Develop preventive and detection controls that reduce risk priority scores.
  • Maintain a live FMEA document as a design or process changes over time.
