Incident and Accident Report Writing for Safety Teams

When an incident occurs on an African worksite, the written report becomes a legal record, a learning tool and often the basis for corrective action. This course teaches safety and operational staff to document incidents factually, completely and without bias. Delegates practise separating fact from assumption, describing sequences of events clearly and writing reports that stand up to investigation and review.

What you will be able to do

  • Record the facts of an incident accurately and in sequence.
  • Separate observed facts from opinion and assumption.
  • Capture the who, what, when, where and how completely.
  • Describe contributing factors without assigning premature blame.
  • Write clear, actionable corrective and preventive recommendations.
  • Produce a report that supports investigation and legal scrutiny.
