Most hazard identification studies fail quietly. The workshop runs, a spreadsheet fills up, and six months later nobody can say which actions were closed. The problem is rarely the method. It is that the study is treated as a document to produce rather than a decision to make.
Get the right people in the room
A HAZID needs the people who operate the plant, not only those who manage it. The operator who has watched a pump cavitate for two years will surface risks that no drawing shows. Keep the group small enough that everyone speaks, usually six to ten people, and make sure at least one has the authority to commit to an action on the spot.
Describe the hazard, not the label
Vague entries like human error or equipment failure cannot be actioned. Push each item until it describes a specific situation: what releases the energy, who is exposed, and what the consequence would be. A good entry reads like a short story with a bad ending, and that is exactly what makes the control obvious.
Rank ruthlessly, then stop
Every hazard feels urgent in the room. Force a ranking so the team spends its effort on the few that would actually hurt someone, rather than spreading it thin across dozens of low consequence items. A study that produces five actions that get done beats one that produces fifty that do not.
Assign, date, and follow up
Each action needs a named owner and a date, captured before anyone leaves. The single biggest predictor of whether a HAZID changes anything is whether someone checks back on those actions. Put that review in the diary during the workshop itself.
